Complete the following attached items which were sent home with your student and return by May 17, 2023 to be registered for the 23-24 School year.
1- Student Comprehensive Report
· Check all the information. (If your address has changed see below.)
· Make corrections in print.
· Sign the Form
· Note- Pay attention to Transportation Information. What is on this form will be how your child is transported to/from school. You may only have one AM and one PM method. (WES ONLY- Car rider numbers will change. New numbers can be picked up at Meet the Teacher in August.)
2- Residency Verification Form
· Complete the form.
3- Medical Paperwork
· Complete the form.
4- Student Permission Report
· Check the Information.
· Make corrections in print.
· Sign the Form.
· Note- True means Yes and False Means No. (Example- If you give permission for your child to be featured on the internet, Web Publicity should have True under it.)
IF THIS PAPERWORK IS NOT COMPLETED AND RETURNED BY MAY 17, 2023, YOUR CHILD WILL NOT BE REGISTERED AND YOU WILL NEED TO ATTEND SUMMER REGISTRATION.
Process for Registration if you have a NEW STUDENT, ADDRESS CHANGE, or AFFIDAVIT:
· Attend Summer Registration on June 21 or June 22, 2023 at your students school (WHS or WES).
o Time: 8:00 a.m.-4:00 p.m. June 21, 2023
o Time: 10:00 a.m.-6:00 p.m. June 22, 2023
· New Students Will Need:
o Birth Certificate
o 121 Form (shot records)
o Social Security Card and/or number
o (2) proofs of residency
o Last report card (if coming from another school)
· Students with Residency Changes will need: 2 proofs of residency (see approved documents on the Residency Verification Form 2023-2024)
· Students attending WMCSD on Affidavits of Residency will need:
o 2 proofs of residency of who the family is residing with. Proofs must be dated within 60 days of the first day of the 23-24 school year.
o Driver’s license for both parties (Parent/Guardian & person they are living with)
o Parent/Guarding & the Person they are living with will have to both sign the 2023-2024 affidavit
